Application for mortality coverage for your horse, including colic surgery coverage.
The insurance company will accept a health statement completed by you on horses to be insured at $25,000 or less and foals over 90 days of age. The health statement will ask specific questions regarding the health history of the horse to be insured. This report will save you the expense of an insurance veterinarian examination.
To be filled out by your Veterinarian. A current examination is required to be completed by your veterinarian for horses valued in excess of $25,000; foals under 90 days of age; all horses used for racing; and may be required for horses with prior health conditions including previous surgery, lameness, illness or injury. Mortality insurance on senior horses may also require a current vet examination. The veterinary report cannot be more than 30 days old at the time your policy begins.
To be filled out by your Veterinarian. A current examination is required to be completed by your veterinarian for foals under 30 days of age.
